1. Complete the  ONLINE APPLICATION or the hard copy form available as a PDF document.  Please include the faculty letter of recommendation with your hard copy materials (application deadline is March 10, 2008).

2. Additional materials need to be sent to the contact address (please include your Username, applications must be postmarked by March 10, 2008).

Download the Faculty Letter of Recommendation for your faculty reference to complete. The letter should be enclosed in a sealed envelop, signed by the faculty on the flap. The sealed envelop may be sent directly from your Faculty Reference to the address on the form, or you return it with your application materials

An official copy of your college/university transcript.

Your Personal Statement. This should be approximately 500 words (no more than 2 double spaced pages), describing your interest in, preparation for and/or commitment to your academic pursuits and reasons for applying to the MTBI Summer Research Program.
